Bridges to Success Strengths Case Management for Homeless Adults with Substance Abuse Disorder (SUD), Serious Mental Illness (SUI), or Co-occurring Disorder (CD) (Bridges to Success) Program provides Strengths Case Management Services to individuals in Josephine County, Washington. Bridges to Success is a five-year grant funded by the Substance Abuse and Mental Health Services Administration. The Bridges to Success program received its grant award in July 2005 and is funded through June 2010.

Bridges to Success Strengths provides Case Management a model based on consumer self-determination, accessing community resources while focusing on individual strengths, and effective engagement and relationships with homeless adults. This five-year project will expand Josephine County, Oregon's capacity to provide outreach among homeless adults with SUD, SUI, or CD in the county, helping them to establish a clean and sober lifestyle, improving their physical health and the quality of their lives, and reducing episodes of criminality, homelessness, and psychiatric crisis.

The Regional Research Institute at Portland State University is conducting a comprehensive process and outcome evaluation of the Bridges to Success program. Over the project period, the RRI will evaluate the programs effectiveness, disseminate program outcomes and develop a manualized approach to treatment of homeless adults with SUD, SUI , or Co-occurring mental health disorders.
